Description (Français)Le tramadol est un analgésique central ayant une activité proche de celle de la codéine, cependant il est un analogue de celle-ci. On le classe dans la catégorie des analgésiques de type 2. Il agit sur le même type de récepteur que la morphine, c’est un agoniste des récepteurs morphiniques, il ne limite pas l’action d’autres morphiniques. Il peut entraîner une dépendance mais celle-ci semble plus faible que celle d’autres morphinomimétiques, tout en restant nettement supérieure aux analgésiques dits de type 1. À ce titre il convient de l’employer avec une toute autre réserve.
Ses modes d’actions ne sont pas encore complètement connus, en plus de son action analgésique due au fixement sur les récepteurs morphiniques, il semble également empêcher la recapture de la sérotonine et de la noradrénaline, selon les doses utilisées.
Il peut être utilisé par voie veineuse. Les doses utilisées sont de 50 à 100 milligrammes. Sa demi-vie est de cinq à sept heures, son élimination est essentiellement rénale.
Les effets secondaires sont : nausées, vertiges, douleurs d’estomac, hypoglycémies, anxiété, crise d’angoisse, dépression (sur le long terme). Une dose trop importante de Tramadol (plus de 400 milligrammes en une prise) peut entraîner un collapsus suivi de spasmes et contractions musculaires importantes, la crise ressemblant symptomatiquement à l’épilepsie. Il n’est pas recommandé de prendre plus de 400 milligrammes par 24 heures. La prescription sera faite avec grandes précautions en cas de traitement par antidépresseurs.
La pharmacodépendance est importante, la posologie sera adaptée en conséquence pour éviter un sevrage difficile impliquant des notions d’hypertension, hypersudation, insomnie, terreurs nocturnes, angoisses, agoraphobie, troubles de la concentration, spasmes musculaires, micro-mouvements involontaires, agressivité, irritabilité, amnésie temporaire (parmi les plus fréquents).
Le tramadol est utilisé abusivement dans un contexte toxicomaniaque, au même titre que la codéïne.
Le tramadol est fortement déconseillé en cas de pathologie hépatique sévère.

Ultram is in the FDA pregnancy category C. This means that it is not known whether it will be harmful to an unborn baby. Do not take this medication without first talking to your doctor if you are pregnant. It is also not known whether Ultram passes into breast milk. Do not take Ultram without first talking to your doctor if you are breast-feeding a baby. If you are over 75 years of age, you may be more likely to experience side effects from Ultram. The maximum daily dose of Ultram for people over 75 years of age is 300 mg. Ultram is not approved by the FDA for use by children younger than 16 years of age.
Missed Dose
Take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Possible Side Effects
Although side effects from Ultram are not common, they can occur. The most frequently reported events were in the central nervous system (Migraine, Speech disorders) and gastrointestinal system (Gastrointestinal bleeding, Hepatitis, Stomatitis, Liver failure). Tell your doctor if any of these symptoms are severe or do not go away: dizziness, headache, drowsiness, blurred vision, upset stomach, vomiting, and diarrhea. If you experience any of the following symptoms, call your doctor immediately: fast heartbeat, redness, swelling, and itching of the face, numbness or tingling of the hands and feet, difficulty breathing, changes in urination, seizures.
